The Addiction Medicine Advanced Practitioner Training Program will provide integrated mental health and substance use disorder (SUD/OUD) services, including Medications for Opioid Use Disorder (MOUD), for physician assistants and advanced practice nurses in a primary care underserved community-based setting.
The overall goal of the APP Trainee Program is to foster robust clinical training and augment expertise among clinicians who will see patients at access points of care and provide mental health and addiction prevention, treatment, and recovery services.
Job Duties:
- Participate in MOUD in community-based settings along with additional clinical training beyond the didactic component, including professional peer-to-peer support to assist trained APNs and PAs in initiating their provision of MOUD.
- Connect and manage patients receiving MOUD and connect them to behavioral health counseling and social supports to retain patients in care.
- Collaborate with caregivers inside and outside of the immediate care team.
- Demonstrate effective communication with patients and the immediate care team.
Responsibilities include:
- Initial and ongoing assessment of patient's medical, physical, and psychosocial status.
- Perform history and physical examination.
- Develop assessment and treatment plan.
- Participate in clinical patient care.
- Record progress notes.
- Order tests, examinations, medications, and therapies.
- Arrange for discharge and aftercare.
- Write and dictate admission notes, progress notes, procedure notes, and discharge summaries.
- Provide patient education and counseling covering health status, test results, disease processes, and discharge planning.
- Perform procedures as needed.
Education:
Masters Degree - Nurse Practitioner required
Certification(s) and License(s):
Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania; Certified Nurse Practitioner - American Academy of Nurse Practitioners
