MAJOR DUTIES AND RESPONSIBILITIES:
Geisinger Care at Home APP is a field-based patient direct care position performing health risk assessments, depression screening and addressing care gaps for Geisinger Health Plan Gold members in Lycoming county, Pennsylvania.
ADDITIONAL INFORMATION:
- Work is typically performed in the patient’s home.
- Approximately 5-10% of the visits are virtual telemedicine.
- APP must reside in geographic area due to nature of position.
- Schedule can be 5 – 8-hour days or 4 – 10-hour days.
- There is no call, weekend, or holiday work.
Advanced Practitioners receive the following benefits:
