Nurse Practitioner or Physician Assistant Family Medicine
We are seeking an experienced Nurse Practitioner or Physician Assistant to join our Primary Care team at the Medical Clinic Buckhorn in Bloomsburg, PA.
Job Summary:
The successful candidate will work under the responsibility and supervision of a physician and require completion of an ARC-PA approved physician assistant program, current NCCPA certification or eligibility, and Pennsylvania State licensure.

Responsibilities:
- Daily huddles with care team members to review coordination of care
- Care managers, health managers, clinic health associates and coding educators
- Nurse call center to assist with on-call workload (nursing home calls directed to Skilled Nursing Facility team)
- Epic EMR universal throughout Geisinger
- Increased access to care by means of eVisits, patient portals, Geisingers ConvenientCare urgent care clinics
