Program Highlights:
The AHCaH program utilizes remote patient monitoring, advanced technology, and interdisciplinary collaboration to deliver exceptional care to patients who meet hospital admission criteria.
Target Population & Conditions Treated:
- Med/Surgical level of care
- Congestive Heart Failure (CHF) exacerbation
- COPD exacerbation
- Urinary Tract Infections (UTIs)
- Other appropriate conditions
Your Role as an Advanced Practitioner (AP):
As the core provider on the AHCaH team, you’ll work closely with patients and collaborate with a dynamic interdisciplinary care team under physician guidance. Key responsibilities include:
- Conducting in-person and remote patient assessments
- Developing and implementing treatment plans
- Providing ongoing monitoring and coordination with field staff
Team Composition:
- Advanced Practitioner (AP): Primary provider managing patient care
- RN Coordinator: Care coordination and program oversight
- Field Staff: Mobile paramedics and RNs for in-home care delivery
- Physician: Direct care and interdisciplinary team supervision
Location:
Based at the Geisinger Healthplex CenterPoint office in Pittston, PA, with limited travel to Geisinger Wyoming Valley, Geisinger South Wilkes-Barre, in Wilkes-Barre and Geisinger Community Medical Center, Scranton as needed.
Schedule:
- Work-life balance: Enjoy a 7 days on / 7 days off schedule
- Hours: 7:00 AM – 7:00 PM
- Call Rotation: Shared with physicians and other Advanced Practitioners

Qualifications:
- Nurse Practitioner or Physician Assistant with relevant certification/licensure
- Clinical experience in acute care or hospital-based medicine preferred
- Exceptional communication and organizational skills
