Requirements
To be successful in this role, you must have at least 5 years of primary care experience and be able to see/treat all age ranges from pediatric to geriatric. Additionally, the successful candidate will require completion of an ARC-PA-approved physician assistant program, current NCCPA certification, or eligibility, and Pennsylvania State licensure for PAs, or a certificate of completion from an approved program for nurse practitioners or eligibility to hold national certification allowing NP to work with patient populations and location settings associated with this position.

- Conducting 20-minute appointments and forty-minute appointments for elderly patient populations
- Participating in a panel sharing model to optimize patient care
- Daily huddles with care team members to review coordination of care
- Collaborating with pharmacists to assist with patient education and medication management
- Working with care managers, health managers, clinic health associates, and coding educators
- Utilizing centralized prescription refill teams and nurse call centers to enhance patient care
- Navigating Epic EMR systems universally throughout Geisinger
- Increasing access to care through eVisits, patient portals, and Geisinger's ConvenientCare urgent care clinics
