Join Our Hospice Care Team! Licensed Practical Nurse (LPN/LVN)
Per Diem - After Hours, Nights, Weekends- San Bernardino/Beaumon/Rednalds/Yucaipa Areas.
About the Role
Are you a compassionate LPN/LVN ready to make a real impact in patient care? As part of our dynamic healthcare team, you'll provide top-notch care following physician orders, industry best practices, and our super supportive company policies. You'll be at the heart of creating a safe, social, and positive environment for our patients.
What You'll Do:
- Be a patient champion! Ensure their rights are respected and they receive the best care possible.
- Keep things organized by completing documentation on the same day (we love efficiency!), and have it in the EMR within 24 hours.
- Conduct EOBs, gather required consents, and submit to intake ASAP—teamwork makes the dream work!
- Supervise and guide nursing assistants (where state regulations allow).
- Administer meds and treatments with a smile and ensure everything is on point.
- Monitor how your patients respond to care and meds—your attention to detail saves the day!
- Partner up with the RN case manager or Clinical Service Manager if something seems off—communication is key.
- Stay in the know with regular training on healthcare trends (because learning never stops!).

What We Need From You:
