Position : Senior Doc Team Coordinator LVN

Location : Orange County, CA

Reports To : Director of Patient Care Services & Team Coordinator Manager

Pay Rate : $35 - $40 per hour, depending on experience

Senior Doc is looking for a Senior Doc Team Coordinator LVN to join our dynamic, caring team, where we prioritize holistic care and meaningful connections with our patients and their families.

What You'll Be Doing :

As our Senior Doc Team Coordinator, you'll play a critical role in ensuring seamless coordination of care for our patients. Your responsibilities will include :

Team Coordination : Scheduling staff, managing care plans, and ensuring effective communication among our multidisciplinary team (MDs, NPs, RNs, SWs, SCs, and more!).

Holistic Care : You'll help develop and manage personalized care plans, working closely with patients, families, and specialists.

Documentation & Compliance : You'll ensure accurate documentation, manage updates to care plans, and maintain compliance with care regulations and quality standards.

Communication & Support : You'll serve as the communication hub, ensuring all team members are aligned and families feel heard, supported, and informed throughout their journey.

Cultural Sensitivity : You'll respect diverse beliefs and practices, ensuring care plans reflect cultural preferences and enhance patient comfort.

Quality & Improvement : Engaging in continuous improvement initiatives, you'll help refine care practices to maintain the highest standards of care.
