JOB SUMMARY :
The Referrals Program Coordinator at Gellert Health plays a key role in supporting our mission to deliver exceptional Health Navigation services with kindness, integrity, and quality. Reporting to the Director of Navigation, this is a frontline position that focuses on acquiring new patients, ensuring a smooth referral process, and providing a positive onboarding experience.
The role involves maintaining strong relationships with referral partners, insurance companies, and other stakeholders, while effectively communicating our mission, services, and criteria via phone, virtual platforms, and in-service presentations.
Ideal candidates are proactive, enthusiastic, and solution-oriented, with excellent client service and communication skills. Experience in sales and marketing or patient acquisition role in the healthcare-related field is preferred. A passion for building positive, collaborative relationships is essential.
KEY RESPONSIBILITIES :
1. Patient Acquisition & Onboarding (Referrals Management)
- Manage daily call lists and outreach, including warm and cold calls, to potential patients who may qualify for our services.
- Educate potential patients about Gellert Health’s Navigation Services and how they can benefit.
- Conduct routine visits to partnering hospitals and organizations to engage in patient acquisition.
- Respond promptly to all inquiries via phone, text, and email, and keep accurate documentation in the workflow system.
- Ensure program eligibility requirements are met and all necessary documentation is completed.
- Oversee the referral process from initial contact to assignment or closure of the referral, ensuring a smooth experience for patients and referral partners.
- Collaborate with the Navigation Team to ensure seamless operation of case assignments and intake scheduling.
- Complete necessary clerical and administrative duties, such as data entry and reporting, coordinating appointments, and patient setup in the EMR, Advanced MD.

PROBATIONARY PERIOD :
This position includes a standard 90-day probationary period to assess performance and fit within the company. In certain cases, this period may be extended at the discretion of management to provide additional time for evaluation or training.
