Position: Case Manager
Regional Health Systems is Northwest Indiana’s largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children—both uninsured and insured—integrated care through our community mental health center and federally qualified health center. Regional Health Systems is a member of the Regional Care Group.
Position Purpose:
To provide case management and skills training to clients who present in the crisis unit. Must be willing to transport clients when needed and travel into the community to access and establish relationships with community stakeholders. Would include making referrals to resources, housing, community resources. Provide case management and skills training to clients who present in the crisis unit. These services are directed toward: providing the skill and support necessary to enable clients to function in the least restrictive environment with the highest possible level of functioning. Case managers provide assessment, service planning, coordination and linkage and monitoring functions. Handles crisis situations for clients who present at risk when needed. Position requires travel in personal vehicle in the community and the homes of clients, as well in the office.

Requirements:
· Bachelor’s degree in social work, Psychology, or related field
· One year of experience with the target population (adult & mental health)
· Driver’s License & Insurance and Clean Driving Record
Shift Details: Full-time – Non-exempt: 9a to 5p, and 11a – 7p depending on the needs of the department.
