What are the responsibilities and job description for the Fatherhood Family Advocate position at Geminus & Regional Health Systems?
Geminus Corporation partners with community-based health care providers, social service organizations and early childhood education programs to help increase access to high-quality social health care for children and families across Northern Indiana. Geminus Corporation is a member of the Regional Care Group.
Scope: The Fatherhood Family Advocate reports to and receives general work direction from the Fatherhood Program Manager. No positions report to the incumbent. The Fatherhood Family Advocate is expected to present a positive and professional image of Geminus, at both the Lake and LaPorte County offices.
Position Purpose:
To provide case management services to children and families in the Division of Child and Family Services through the Family Reunification program.
- Provide home based services.
- Develop safety and risk assessment.
- Facilitate child and family team meetings.
- Develop service plans.
- Provide documentation through computer programming.
- Attend court; provide written reports and provide court testimony as needed.
- Complete client surveys to assess cases.
Essential Competencies:
- Provide excellent customer service/communication skills.
- Demonstrate ability to utilize computer skills including Microsoft Office/Outlook, data entry and computer program applications as required.
- Detailed oriented.
- Be a team-player.
- Strictly adhere to program policies and procedures.
Minimum Hiring Requirements:
Education: BA/BS in social work, psychology or related field
Experience: Minimum of 2 years experience working with youth and families providing home based services providing child welfare services.
Licensure and Certification: None required
Shift: Full-Time Days
