In addition, Gemma offers referral bonuses, education assistance and self-care funds for each program to participate in wellness activities.Job Specifics : Provide direct service to a caseload of families as specified in the DHS Scope of Service and participates in the initial joint visits with DHS for families. Conduct weekly home visits in each family's homes. Communicate with families, schools, medical offices, behavioral health services, and other agencies for each family for the purposes of resource development, continuity of care and data gathering. Have knowledge and understanding of the social welfare process. Ensure accurate and timely completion of all documentation in the electronic health records (PFDS & Evolv), Coordinate case openings, closings, dismissals, and discharge planning. Complete initial assessments and summaries.Ideal candidates must reside in Philadelphia, PA or surrounding Tri-State areas. We are also interested in hearing from you if you are willing to relocate to the Philadelphia, PA area. Must have a valid driver's license.Education / Training : Family Empowerment Case Managers are required to have a B.A. / B.S. in Social Work or a closely related field (i.e., social services, sociology, or psychology) from an accredited college / university.Skills(s) : Proficient Microsoft Office software, Electronic Medical Records, customer service principles and practices; problems solving ability and the ability to interact sensitively with children and families.Experience : At least two (2) years of experience working with children and / or families.
