Gen X Consumer Solutions LLC is seeking a Patient Care Operations Coordinator (PCOC) to oversee home health patient scheduling. This role ensures efficient coordination between clinicians, home health aides, and management to maintain timely and effective patient care.
Key Responsibilities
- Manage home health patient schedules in collaboration with clinicians, aides, and managers.
- Ensure schedules are completed in advance and updated as needed.
- Communicate schedule changes to clinicians, aides, and patients.
- Ensure adequate clinician and aide scheduling to meet productivity goals.
- Assign patients and services in alignment with timely access to care requirements.
- Coordinate and complete daily/weekly reports to ensure patient safety.
- Maintain accurate records of employee profiles, availabilities, schedules, and patient demographics.
- Process payroll reports and perform data entry.
- Escalate scheduling conflicts to management.
Qualifications
- High school diploma or GED required; Associate degree preferred.
- Previous experience in scheduling or home health care preferred.
- Strong organizational and communication skills.
