Position Overview: We are seeking a licensed practical nurse (LPN) with a minimum of two years of intake experience and proficiency in insurance verification. The Intake Coordinator will be responsible for managing patient referrals, coordinating with healthcare providers, and ensuring a seamless admission process for our patients.
Key Responsibilities:
- Evaluate and process patient referrals from various sources.
- Verify insurance coverage and benefits for home health and hospice services.
- Coordinate with healthcare providers to obtain necessary documentation and approvals.
- Provide information and support to patients and families regarding services and coverage.
- Maintain accurate and up-to-date records of all intake activities.
Qualifications:
- Current LPN license.
- At least 2 years of experience in intake coordination, preferably in home health or hospice settings.
- Strong knowledge of insurance verification and coverage assessment.
- Excellent communication and interpersonal skills.
- Proficiency in electronic medical records and office software.
