What are the responsibilities and job description for the RN Case Manager - Home Health position at Generation Home Health?
As a home health care case manager, your job is to provide hands-on patient care within a patient’s home. Your responsibilities include developing a Plan of Care (POC) for patients based on their needs and helping support adherence to the treatment plan. Additional duties include coordinating the implementation of the plan with the primary doctor, making changes as needed, and completing all required documentation.
What we’re looking for:
- Licensure: Current license to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Ohio.
- Current CPR certification.
- Valid state-issued driver’s license, automobile insurance, and reliable transportation.
