RN – Ashe Assisted Living & Memory Care / Generations Adult Day Care
Primary Responsibilities-Ashe Assisted Living & Memory Care Resident Health & Wellness :
1. Review all resident information and perform resident assessment and care plan within 30 days of admission; within 10 days of a significant change and / or annually and quarterly for Special Care Unit residents.
2. Participate in the on-site review and evaluation of the residents’ health status, care plan and care provided for residents requiring one or more LHPS (Licensed Professional Health Support) tasks. The evaluation and documentation must be completed within the first 30 days of admission or within 30 days from the date a resident develops the task and at least quarterly thereafter.
3. Meet with each resident on a regular basis to provide health and wellness checks, including any skin / wound care, and answer any health related questions.
4. Maintain ongoing communication with the Resident Care Coordinator, Memory Care Coordinator and Administrator.
5. Assist with coordinating additional health services the resident may need.
6. Assist in coordinating resident transfer to and from a hospital or another facility, if needed.
7. Assist with counseling residents and families, if needed.
8. Be on call for medical questions and resident emergencies as scheduled.
9. Review changes in residents’ conditions with the Administrator, Resident Care Coordinator and / or the Memory Care Coordinator.
10. Assess residents and monitor their health.
11. Assist in ensuring that all infection control guidelines are met.
Management
1. Assist Administrator in implementing state regulations as required.
2. Assist Administrator with developing and / or changing resident care policies and procedures as needed.
3. Complete all agency and state required documentation in a timely manner.
4. Function as part of the management team (participate in meetings and facility events)
Nursing
1. Conduct resident re-assessments as outlined by licensure guidelines.
2. Develop and implement resident care plans to meet needs of residents per agency policies and state licensure guidelines.
3. Provide nursing support to Resident Care Coordinator and nursing oversight and support to Memory Care Coordinator.
Resident’s Rights
1. Assist with ensuring compliance and understanding of all regulations regarding residents’ rights at all times and report observed violations.
Training and Regulatory Compliance
1. Assist with training regarding residents’ health and wellness, to include but not be limited to LHPS competency training, med-administration competency, diabetes training, and infection control.
Primary Responsibilities-Generations Adult Day Care
1. Coordinate the delivery of Health Care services and participate in direct care as needed.
2. Responsibilities include :
a. Completing preadmission health assessment for initial acceptance into program, including problem-identification and care planning
b. Implementing the health care components of the established service plan which shall include :
1) Medication administration;
2) Wound care
3) Enteral or parenteral feedings;
4) Bowel or bladder training and maintenance programs;
5) Tracheostomy care and suctioning; and
6) Delegating nursing care tasks to qualified unlicensed personnel;
c. Monitoring participants response to medical treatment plan and nursing interventions and revising plan of care as necessary
d. Reporting and recording results of the nursing assessment
e. Collaborating with other health care professionals and caregivers regarding provision of participants health care;
f. Educating other staff members to emergency procedures and providing information to staff and caregivers about health concerns and conditions of participants;
g. Providing first aid treatment as needed;
Knowledge & Skills
1. Minimum one year experience, in health care setting or long term care setting
2. Have current standing as Registered Nurse with the NC Board of Nursing.
3. Experience and knowledge of medication administration procedures.
4. Experience & knowledge of diagnoses & treatment plans for such diagnoses
