Wound Care Consultant (90% Travel Position)

29 Facilities throughout California

Licensed Nurse and Wound Certified

Full Time $120,000 - $140,000 / Annually DOE

Generations Healthcare is seeking a dedicated (LVN or RN) Wound Care Consultant to join our dynamic team. This role offers the unique opportunity to serve the entire state of California, with a primary focus on wound care. We are looking for a passionate professional with a deep commitment to wound care, who is eager to educate and mentor others, ensuring the highest standards of care and best practices are upheld across our organization.

Position Overview :

The Wound Care Consultant will be responsible for developing, overseeing, and continuously improving a comprehensive wound care program across multiple skilled nursing facilities. This role will work closely with facility leadership, nursing staff, and the interdisciplinary team to ensure residents receive the highest standard of wound care. The Wound Care Consultant will also provide ongoing education, training, and support to staff to mitigate the risk of wounds and improve resident outcomes.

Essential Responsibilities :

Program Development and Oversight :

  • Develop and implement a standardized wound care program for all facilities, ensuring it is consistent with best practices, regulatory requirements, and evidence-based standards.
  • Monitor the implementation and compliance with the wound care program at each location, ensuring proper documentation and effective interventions.
  • Conduct periodic reviews and assessments of the wound care program's effectiveness and make adjustments based on feedback and outcomes.

Wound Assessment and Management :

  • Conduct regular visits to each facility to assess residents with existing wounds, providing advanced wound care expertise to nursing staff.
  • Assist in the management of complex or chronic wounds, including pressure ulcers, diabetic foot ulcers, and surgical wounds.
  • Provide recommendations for individualized care plans for residents with high-risk wounds or at risk for developing wounds.
  • Education and Training :

  • Design and deliver ongoing training programs for facility nursing staff on wound prevention, assessment, treatment, and documentation.
  • Educate staff on the latest advancements in wound care, including new technologies, products, and treatment modalities.
  • Empower facility leadership and clinical teams to proactively manage and prevent wounds, fostering a culture of quality care and accountability.
  • Provide hands-on education and training in wound care techniques, dressing changes, and proper use of equipment.
  • Risk Mitigation :

  • Conduct assessments to identify potential risks for wounds among residents, including pressure injury risk assessments and skin assessments.
  • Develop strategies and tools to prevent the development of wounds, such as repositioning schedules, nutritional assessments, and skin care protocols.
  • Collaborate with facility teams to ensure proper use of wound prevention equipment, such as pressure-relieving mattresses and positioning devices.
  • Quality Improvement :

  • Collect and analyze wound care data from each facility to monitor trends, identify issues, and implement corrective actions as needed.
  • Track and report wound care outcomes and metrics to senior leadership, providing recommendations for program improvement.
  • Stay informed on industry trends and regulatory changes related to wound care to ensure the program remains compliant and up to date.
  • Collaboration and Support :

  • Collaborate with the medical director, facility administrators, and interdisciplinary teams to ensure a comprehensive approach to wound care.
  • Serve as a resource for physicians, nurse practitioners, and clinical staff regarding wound-related issues and advanced treatment options.
  • Provide ongoing support to facility leadership to ensure continuous improvement and quality care in the management of wounds.
  • Knowledge, Skills, and Abilities :

  • Strong knowledge of wound care best practices, clinical guidelines, and regulatory standards.
  • Strong knowledge of skilled nursing practices and principles including CMS, Title 22, and other regulatory requirements related to wound care.
  • Strong knowledge of Electronic Medical Records; PointClickCare (PCC) preferred.
  • Passionate about resident-centered care and committed to improving health outcomes through proactive wound prevention and management.
  • Strong communication and interpersonal skills to effectively train and educate staff at all levels.
  • Strong proficiency in computer skills, including knowledge of Microsoft Office Suite or similar program.
  • Proven success in providing respectful and timely service to all internal and external parties.
  • Unquestionable integrity, confidentiality, and performance of duties with dignity and compassion.
  • Proven ability to assess systems and processes and make improvements.
  • Ability to speak, read, and write the English language.
  • Proven success in leading and working in a team-oriented setting, utilizing others' skills and knowledge, sharing information, and supporting team outcomes.
  • Proven ability to successfully work with and meet deadlines.
  • Ability to work independently and travel to facilities across the organization.
  • Proven ability to successfully perform in an environment with changing priorities & assignments.
  • Education, Experience and Requirement :

  • Registered Nurse or Licensed Vocational Nurse with a current and unencumbered license required.
  • Wound Care Certification (WCC) or Certified Wound Care Nurse (CWCN) preferred.
  • Minimum of 3-5 years of experience in wound care, wound care education, and wound care risk mitigation, preferably in a skilled nursing or long-term care setting preferred.
  • Experience with quality improvement initiatives and data analysis preferred.
