Key Responsibilities :
Perform skilled nursing duties such as wound care and medication management
Monitor and assess changes in clients’ conditions
Collaborate with interdisciplinary care team members to ensure comprehensive, high-quality care
Educate clients and families about healthcare management options
Maintain accurate documentation of nursing care provided
Requirements :
Valid New Jersey LPN license
CPR / Basic Life Support certification
Proof of eligibility to work in the U.S.
Proficient computer literacy
Valid driver’s license and reliable car
Ability to work independently and as part of a team
Strong clinical and critical thinking skills
Exceptional communication and interpersonal skills
Ability to lift up to 50 lbs
One year of nursing experience preferred
Home care experience preferred
