    Job Summary: The Early On® Service Coordinator is responsible for assessing the needs of children, ages birth to five, and coordinating services to meet the child’s needs. The Early On® Service Coordinator will write Individualized Family Service Plans (IFSP), demonstrate learning, play, and communication activities in the home and provide the family with information related to child development.

    Essential Duties (May include, but not limited to):
    • Responsible for the implementation of Individualized Family Service Plans (IFSP).
    • Meets with the Teacher Consultant for Students with Disabilities, Early Childhood Programs & Services to review and assess individual family needs.
    • Arranges and conducts interviews with referred families, establishes rapport with families, develops Individualized Family Service Plan (IFSP) goals, and arranges follow-up meetings times and dates.
    • Conducts home visits to expand the family’s understanding of children’s developmental milestones and demonstrate meaningful play/learning/communication activities for children.
    • Administers the Infant Developmental Assessment (IDA) to assigned children. Scores tests and submits test results to the family and the Early On® office.
    • Writes progress reports, Individual Family Service Plans (IFSP), reviews, parent contact forms, and transition plans on assigned children. Communicates child’s progress to family.
    • Submits completed paperwork to Early On® office in a timely manner (within 10 days of completion).
    • Uses district technology to evaluate, establish, and maintain records of child’s progress.
    • Consults, counsels, and collaborates with parents, school personnel, children, and appropriate community agencies regarding behavioral and educational concerns developed in a multi-tiered system of support.
    • Consistently models Win-Win leadership.
    Other Duties:
    • Participates in Early On® staff meetings as requested by the Teacher Consultant for Children with Early On® services.
    • Performs other related duties as assigned.

    Qualifications:

    Education:
    Bachelor’s degree with concentration in Early Childhood education, elementary education, special education, social work, speech and language therapy, occupational therapy, or physical therapy. Provider who meets the State of Michigan personnel requirements for their discipline or profession or Michigan Association for Infant Mental Health Endorsement® (IMH-E®) preferred.

    Experience:
    Experience working with children, birth to five. Prefer experience working with low-income families in an urban setting. Work experience that demonstrates the ability to work well with community service agencies, complete child development assessments, and conduct home visits. Prefer work experience in child development or related services area.

    Skills/Other:
    Must meet State of Michigan Department of Human Services Child Abuse and Neglect Central Registry Clearance as applicable. Ability to accept supervision and direction. Work as a team member, establish family goals, accurately complete child development assessment, and meet the individualized needs of assigned families. Ability to communicate effectively with families. Ability to utilize district technology and maintain proficiency, as required skills sets change with technology and/or the needs of the district. Ability to organize own workflow to meet state and federal deadlines. Must have a valid Michigan driver license and have reliable transportation for travel to and from homes in the assigned geographical area. Represents the district in a professional manner at all times. Works successfully as a member of the team. Demonstrate honesty, integrity, and professionalism at all times. Exhibit emotional stability and physical stamina necessary for lifting and moving materials from the work site to family homes and meeting the essential elements of the job description. Exhibit an interest and enthusiasm for working with low-income families/children, peers, and Early On® staff. Ability to work a flexible workday, work week, and work year. Punctuality and good attendance are requirements for the position. Exhibit good judgment and make decisions in accordance with board policies and established administrative guidelines.
