As an LPN MDS Coordinator, you will participate in the Resident Assessment Process through the completion of pertinent items on the Minimum Data Set (MDS) and accompanying Care Area Assessments (CAAs). This ensures the provision of appropriate patient care and nursing center reimbursement for the care provided.
Key Responsibilities:
- Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process while following guidelines for completion, computer encoding, and electronic State transmission of the MDS.
- Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes as required by Federal and State regulations.
- Train new members of the interdisciplinary team on clinical content and electronic completion of applicable MDS items.
Requirements:
- You must be an LPN in good standing and currently licensed by the state.
- Prior experience in a hospital, long-term facility, or other healthcare-related facility is required.
- You must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures, and laws, regulations, and guidelines governing long-term care.
- You must have computer skills, including Windows applications.
- AccuMed experience is preferred.
- Previous experience as an MDS Coordinator is preferred.
- Prior experience in completing MDSs and Care Plans is required.
- Demonstrated proficiency in RAI/PPS processes is required.
- You must maintain current BLS/CPR certification (excluding ALFs/ILFs).

Compensation: The posted salary range for this role is USD $32.00 - USD $39.00 per hour.
