Job Title: Patient Care Specialist

This role plays a key part in meeting Genesis business objectives by ensuring appropriate reimbursement for patient care services.

Key Responsibilities:

• Participate in the Patient Assessment Process through completion of items on the MDS and accompanying Care Area Assessments (CAAs)
• Assist in developing individualized patient care plans, identifying patient needs and projected outcomes as required by Federal and State regulations
• Contribute to the nursing center interdisciplinary Resident Assessment Instrument (RAI) process while following RAI guidelines for completion, computer encoding, and electronic transmission of the MDS
• Assist with Activities of Daily Living training for new hires and through ongoing continuing education to existing staff

Requirements:

• Previous experience in a hospital, long-term facility, or other healthcare-related facility is required
• Prior experience completing MDSs and Care Plans is required
• Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and procedures and laws, regulations, and guidelines governing long-term care
