The Program Coordinator reports directly to the CEO. The Coordinator is an essential position at Genesis. The responsibilities are in 3 main categories, Medical / Medication, Supervisor Oversight, and Training / Coaching. This person will provide oversight for the medical appts. and medication updates / changes. Oversight ensures that the doctors recommendations / updates are being followed, specialists and psychiatrists included. With medication changes the coordinator will work with the supervisors (RLS in training) to disseminate & train staff on those changes. This person will keep supervisors and staff up to date on Medication Administration and CPR training. The Coordinator will work with the supervisors to become raters using the Health Risk Screening Tool (HRST) for the individuals in their program. A successful Program Coordinator must be detail oriented, able to follow through on all tasks, possess the ability to problem solve and is an excellent communicator. Theyre a team leader who can accept input from other team members. Must be organized and dependable. Create and train new systems that promote best practices. Ensures superior care is delivered to each individual while working collaboratively with agencies, families, and Supports Coordinators.
Responsibilities
Medical / Medication
MEDICAL / MEDICATION
o Track medical appointments for all GCH individuals
o A member of the admissions team : Review & Assess the medical records of all referrals to ensure each person has all the necessary medical documentation to be admitted.
o Complete / Update annual Lifetime Medical reports for each individual
o Compile / Maintain a list of current medications for each individual
o Review, Audit and Maintain MARs for each program monthly
o Be the liaison between Genesis and the Pharmacy of choice. Capable of finding a new pharmacy if needed.
o Learn and keep the HRST system updated.
TRAINER
o Become / Maintain the Medication Administration train-the-trainer license
o Become / Maintain the CPR Train-the-trainer certification
o Train all staff in medication administration and perform annual practicums
o Train all staff in CPR and perform annual refreshers
o Work with the supervisors (RLS in training) to become raters in HRST
COACHING / MANAGING SUPERVISORS
o Offer guidance and reassurance to the Residential Lead Staff (RLS) to help them become supervisors
o Assist the RLS with time management. Via weekly meetings
o Assist the RLS on compliance with the State Regulations (identifying problems AND solutions)
o Individual meetings with each RLS to provide personalized support
o Quarterly team meetings with the RLS to boost morale. (Mtgs. inside and outside of the office)
ORGANIZATION
o Learn the 6400 Regulations
o Works with the Program Specialist to provide exceptional services to all individuals
o Maintain medical records for all individuals (binders)
o Annual update of all Lifetime Medical (LTM) reports
o Maintain files on OneDrive
o Create and maintain systems that promote best practices
o Stay updated on emails
