Company Update :

The owner of Genesis owns several other companies that provide long-term care in group home settings. To make things simpler, he has decided to combine all of his companies and manage them under one new company called Synova Group. Synova Group will handle employee services, like recruitment, payroll and training and etc.

The House Supervisor position is for our Hermantown location.

JOB SUMMARY :

The primary purpose of the position is to directly supervise the operations of Community Residential Services or Integrated Community Support Settings delivery to individuals who have developmental / intellectual, mental health, or physical disabilities. This position ensures that all services are delivered according to Synova Group / Genesis Group Homes’ policies and procedures, county contracts, support plan (SP), and all applicable federal, state and county statutes, rules and regulations, and policies. This position also provides direct care to individuals as needed. All duties are subject to policy, procedure and management directives, and may be changed at any time.

This position reports to the Services Manager. The position is classified as hourly, non-exempt.

DUTIES AND RESPONSIBILITIES :

Supervision of DSP’s

  • Supervise day to day operations at the site, ensuring DSP’s deliver services according to Synova Group / Genesis Group Homes’ policies and procedures.
  • Monitor all Direct Support Professionals in assessment collection and program implementation.
  • Plan agenda and lead monthly site meeting.
  • Directly supervise and participate in the orientation and training of Direct Support Professionals.
  • Provide performance feedback and coaching to DSP staff as needed.
  • Complete annual performance reviews.
  • Monitor employee compliance training completion to ensure DSP’s meet DHS standards.
  • Manage the schedule, including covering open shifts, based on the needs of the individuals or the home’s staffing budget.
  • Manage EVV data and ensure compliance.
  • Maintain payroll records including timecards, PTO requests, and overnight check-ins.
  • Ensure that all employment standards are met according to federal and state law, and Synova Group / Genesis Group Homes’ policies and procedures.

House Supervision

  • Directly supervise the prudent spending and documentation of Synova Group / Genesis Group Homes’ financial resources, including general budgets, payroll budgets, program accounts and petty cash accounts.
  • Coordinate and directly supervise the provision of nutritionally adequate meals and ensure that food, cleaning, and other supplies in the home are adequate.
  • Coordinate and directly supervise cleaning at the home site to ensure a home-like, attractive appearance both inside and out.
  • Complete monthly site safety checklist, report repair needs, keep sidewalks and steps free of snow and ice, and implement cleaning lists.
  • Maintain all individual records and on-site Synova Group / Genesis Group Homes’ records, periodically audit records to ensure compliance.
  • Person-Centered Approach with Individuals

  • Directly supervise the implementation of individual rights, data privacy, and the grievance procedure.
  • Supervise the promotion of safety and well-being of all individuals at the home.
  • Perform duties related to admission, suspension and termination as assigned by management or Synova Group / Genesis Group Homes’ policies and procedures.
  • Participate in the writing of all bi-weekly, quarterly, annual, or as assigned reviews and reports. Document all services and events.
  • Develop and demonstrate person centered and therapeutic working relationships with individuals, staff, families and friends, management, and other agency personnel.
  • Maintain knowledge of the service satisfaction of all members of the Interdisciplinary Team (IDT) and act to resolve dissatisfaction.
  • Participate in the design of and monitor the implementation of each individual’s One Page Profile, ISP Goals, SMA, IAPP, SPA, including methods, according to 245D, county contracts, SPs, and Synova Group / Genesis Group Homes’ policies and procedures.
  • Enable individuals to make choices in all areas of their lives while balancing the SP, SMA, IAPP, SPA, Program Abuse Prevention Plan, Rights Restrictions, and IDT directives on safety and individual health, legal and societal norms.
  • Encourage use of public resources and recreational facilities; provide community integration opportunities; help to establish and keep community relationships and friendships.
  • Assist individuals according to the SP and SPA with daily routines including, but not limited to, household tasks, cooking and eating, individual self-care, socializing, use of leisure time, health and safety, money management and behavior management.
  • Maintain individual confidentiality and immediately report all issues related to individual rights, safety, or maltreatment to the Services Manager and / or Residential Director.
  • Administer all scheduled medications; complete all required documentation related to medication administration.
  • Inform the RN of changes in medication administration orders and coordinate implementation and documentation of the changes.
  • Deliver first aid, CPR, and emergency medical services; obtain delivery of all emergency medical services during medical incidents or illnesses.
  • Deliver all physician or nurse treatment orders.
  • Make, provide transportation to, and attend medical, dental, specialist, psychiatric, and psychological appointments.
  • Keep health professionals informed about individual health service delivery and health-related incidents.
  • Consult with medical providers and document results; convey important information about incidents, accidents, and illnesses to supervisor.
  • Directly supervise the prudent and safe receipt, handling, storage, disbursement and documentation of individual funds and property.
  • Keep the Services Manager informed about financial issues, personnel issues, individual and company incidents, service delivery results, individual satisfaction, missed deadlines, household or program needs, non-performance of duties, missing money, and illegal activity.
  • Inform the Services Manager and other management staff of individual progress, concerns and incidents.
  • Perform all duties of Direct Support Professionals when assigned coverage hours according to the job description of the position worked.
  • Other duties as assigned.

    WORKING CONDITIONS :

    The House Supervisor is subject to flexible hours, including split shifts, varied days and times, including mornings, evenings, weekends, and overnights, depending on coverage needs of the site. This position will be cross trained at other sites and expected to cover shifts as necessary. This position includes weekend on-call rotation with other site(s).

    This position will travel regularly into the community and work in a variety of settings both inside and outside the site and throughout the community. The House Supervisor is subject to frequent interruptions, crisis management, and imposed deadlines. This person is subject to frequent problem-solving activities and has regular contact with individuals, family members, friends, federal, state or county regulatory personnel, other provider personnel, and the general public, who may be hostile or emotionally upset. The position may be subject to physically aggressive individuals and may be exposed to infectious diseases, odor, and dust.

    JOB QUALIFICATIONS AND REQUIREMENTS :

    MINIMUM –

    High school diploma or its equivalent.

    Three (3) years of experience delivering services to persons with disabilities.

    Six (6) months of service in a DSPIII position or six (6) months of supervisory experience.

    REQUIRED PHYSICAL AND DEMONSTRATED ABILITIES –

  • Move intermittently throughout the day.
  • Lift persons or implement required therapeutic interventions, if required.
  • Read, write, and speak English sufficiently to perform the duties.
  • Possess good health to perform the duties.
  • Meet the licensing, contractual, and other mandated requirements of the state and county.
  • Possess a valid driver’s license and access to an insured vehicle.
  • Make independent decisions when circumstances warrant; demonstrate initiative, creative problem-solving, and adaptability.
  • Demonstrate positive demeanor.
  • Follow daily routines while allowing for individual decision-making and flexibility.
  • Operate a computer, calculator, household appliances, fire systems and other necessary equipment.
  • Possess cell phone or maintain adequate phone access in order to allow for staff contact.
  • Demonstrate a working knowledge of Synova Group / Genesis Group Homes’ policies and procedures related to services and emergencies.
  • Demonstrate a working knowledge of and implement the site’s Program Abuse Prevention Plan.
  • Demonstrate a working knowledge of assigned individual’s history, skills, program plans, One Page Profile, IAPP, SMA, SPA, SP and ensure implementation.
