What are the responsibilities and job description for the Post-Acute Care Manager (RN) Hybrid position at Genesis Medical Associates?
Genesis Medical Associates, Inc. is currently seeking a Full-time Post-Acute Care Manager (PACM) Nurse for a hybrid position in our central corporate office. If you have strong customer skills and a desire to strive for a positive work environment, we encourage you to apply.
The PACN’s job is to engage and support patients and/or caregivers during transitions from an acute care facility (hospital, ER, SNF) into the community. Through medication reconciliation, care coordination, and education, the PACN calls the patient within a timely manner post-discharge to ensure high quality follow up care. The PACN works in a team environment to meet the dynamic needs/goals of a growing population health program.
Education/Experience:
- Must be a licensed Registered Nurse
- 2 years clinical, quality management or healthcare related experience (minimum)
- Must have excellent conversational skills
- Must have basic computer application skills- MS Word, Excel, Web-based applications, etc
- Knowledge/Experience with Electronic Medical Records (preferred)
- Clinical experience requiring direct/telephonic interaction with patients (preferred)
