Summary
The Diagnostic Family Services Coordinator is primarily responsible for working with the families of the youth that are in the diagnostic program. Working with the families to obtain information, complete assessments, schedule and work with the treatment team and psychologist.
Responsibilities
- Maintain professional, appropriate, collaborative relationships with community providers and local WV DoHS, probation, circuit courts, etc.
- Extensive travel throughout your assigned region is required.
- A successful candidate will ensure regular communication among stakeholders including daily documentation, monthly and discharge reporting.
- Must have excellent verbal and written communication skills.
- Must be able to complete assessments of client's families strengths and needs for youth in treatment and service planning.
- Must be available on non-traditional hours.
Qualifications
- At least 21 years of age
- Pass criminal background check and CPS check
- Pass a pre-employment drug-screen
- Have a Degree in Human Service field which includes social work, sociology, psychology, counseling
- Valid Driver's License
