Job Title: MSHO Care Coordination Supervisor
Position Summary: The MSHO Care Coordination Supervisor is responsible for a team of MSHO Care Coordinators and will provide training, ongoing coaching, and be a consistent support to their teams to ensure they have the right tools and knowledge to provide quality care coordination and regulatory compliance. The MSHO Care Coordination Supervisor will work collaboratively with the Care Coordination Manager in efficient process development, staff oversight, and team development. The MSHO Care Coordination Supervisor will also carry a small caseload of members and is responsible for the delivery of high-quality care coordination to these members.
Supervision Received: Care Coordination Manager
Supervision Exercised: MSHO Mentor, Care Coordinator RN, and Care Coordinator LSW
Hours per Pay Period: 80hrs Classification: Full-Time Exempt
Education/Qualifications:
- Must be Registered Nurse (RN) or current Social Work (BSW/MSW) licensure
- Exceptional professional communication and organization skills
- Must have 1 year of home and community-based experience and be a Certified Assessor for the MNCHOICEs program
- Knowledge of Medicare, Medical Assistance, and Federal Waiver programs
- Knowledge of geriatric population and special care needs
- Must be willing to work with ethnically diverse populations
- Proficient computer skills with specific knowledge of Microsoft Office programs
- Willing to work independently from a home office as well as function as a team member
- Ability to set priorities and manage stress effectively
- Must have proven ability to lead in all aspects of the care coordination infrastructure with the confidence and ability to support and supervise other members of the team
Supervisor Essential Functions:
- Provide ongoing support and education to the team to ensure high-quality care coordination and regulatory compliance
- Collaborate with Care Coordination Manager to identify performance improvement initiatives
- Collaborate with Care Coordination Manager and Human Resources in the interview/hiring process
- Train and mentor new Care Coordinators, Support Specialists, and Transition team members as assigned. Providing essential knowledge and tools to ensure success and regulatory compliance
- Prepare and present efficient small and large group meetings
- Evaluate employee performance thru chart audits, 1:1 meetings, and peer/mentor feedback to create constructive, actionable, and objective evaluation
- Collaborates with Care Coordination Manager and other stakeholders to write and update policies and procedures for care coordinators, support specialists, and transition team to reflect regulatory changes in a timely manner
- Collaborate with Care Coordination Manager and Human Resources to create and evaluate performance improvement plans (PIPS) for staff improvement needs.
- Assist with monthly member assignment as directed by Care Coordination Manager
- Organize and prepare for staff leave
- Monthly MMIS entry and Part C reporting for team
- Collaborate with Care Coordination Manager in addressing member, provider, or other grievances
- Provide case review and direction as needed i.e. high-risk cases, AMA discharges, etc.
- Attend health plan meeting, review communications from all regulatory entities
- Work collaboratively with all members of the MSHO team

- Other functions as determined by management team. Performs other related duties as assigned
Care Coordinator Essential Functions:
- Maintain a caseload, with the following duties:
- Provides comprehensive assessment or screening of each member’s social, medical, and environmental safety needs. Documents results in a timely manner on appropriate form
- Conducts visits in person and by telephone on a timely basis
- Performs review of medical records in the appropriate EMR
- Develops Person Centered Plans of Care based on assessment maximizing the level of self-determination and member choice of services, service providers and living arrangements to meet identified needs. This includes disease management, advance directive planning, and vaccination updates.
- Incorporates interdisciplinary/holistic and preventive care across all settings of care per CMS timeline
- Provides culturally appropriate care
- Communicates care needs and plan of care with member and/or member representative, primary physicians and interdisciplinary team members through written and/or verbal communication
- Contacts appropriate personnel in hospital, nursing facility, foster care or assisted living settings to share and obtain necessary information that will facilitate discharge of the member to the appropriate level of care
- Communicates with service providers and members while assisting with arrangement and authorization of services and issue denials of inappropriate services in a timely manner
Physical/Mental Demands and Work Environment:
- Requires sitting and standing associated with a normal office environment.
- Requires the use of office equipment, such as copiers, computers, telephones, and fax machine
- Work requires hand dexterity for office machine operation, stooping and bending to files and supplies, mobility to complete patient visits, or sitting for extended periods of time
- Able to handle stress in dealing with tense, angry, and/or upset clients, patients, or staff.
- Must be able to prioritize activities when faced with competing demands
- Must be able to lift 25 lbs. occasionally, 10 lbs. frequently
- Use of personal vehicle. Vehicle must be reliable and have proof of insurance. Employee must have a valid driver’s license.
