Responsibilities
- Provide pharmaceutical care to patients in an outpatient setting
- Develop and implement individualized treatment plans
- Conduct patient assessments, monitor medication regimens, and adjust as necessary
- Collaborate with healthcare teams to ensure comprehensive care
Requirements
- Current licensure as a Registered Pharmacist (RPh)
- Experience working in an outpatient pharmacy setting
- Excellent clinical skills and knowledge of medications
