Job Overview
We are seeking a dedicated and compassionate Case Manager Registered Nurse (RN) to join our team. In this role, you will coordinate patient care to ensure the delivery of high-quality, cost-effective services while optimizing patient outcomes. You will collaborate with healthcare providers, patients, and families to develop individualized care plans and facilitate access to essential medical and community resources.
Key Responsibilities
- Conduct comprehensive patient assessments to identify healthcare needs.
- Develop and implement individualized care plans with input from patients, families, and the interdisciplinary team.
- Coordinate patient care across settings, including hospitals, outpatient facilities, and home health services.
- Monitor and evaluate care plans, adjusting as needed to meet patient goals and outcomes.
- Advocate for patients to ensure timely access to appropriate care and resources.
- Provide education and support to patients and families about treatment plans, medications, and services.
- Collaborate with insurance companies and payers to secure necessary treatment coverage.
- Maintain accurate, timely documentation in compliance with regulatory and organizational standards.
- Participate in quality improvement initiatives and case review meetings.
Qualifications
- Current and unrestricted Registered Nurse (RN) license in Virginia or compact license.
- Acute care hospital inpatient experience required.
- Strong knowledge of healthcare delivery systems, community resources, and payer systems.
- Proficiency in Epic electronic medical records system.
- Exceptional organizational, communication, and problem-solving skills.
- Ability to work both independently and collaboratively in a fast-paced environment.
Shift:
5x8 Hr. Days (40 Hrs. per Week)
Duration:
13 Weeks
Pay Breakdown:
$2,200.00 Gross ($720.00 Taxable & $1,480.00 Tax-Free) ($55.00/Hr. Blended Rate)
