">We are seeking a skilled Long Term Care nurse to join our team for a 13-week travel assignment in New York. As a Long Term Care nurse, you will have the opportunity to work with patients who require ongoing medical care and support in a long-term care setting.
">Responsibilities
">- Provide high-quality patient care, including assessing patient needs, developing individualized care plans, and implementing interventions to promote optimal health outcomes.
- Collaborate with other healthcare professionals to coordinate patient care and ensure continuity of services.
- Monitor patient progress, report changes or concerns to supervisors, and make adjustments to care plans as needed.
- Maintain accurate and detailed records of patient interactions, treatment plans, and outcomes.
Requirements
">- Degree in Nursing (LPN/LVN)
- Current licensure to practice as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in New York
- At least one year of experience in Long Term Care nursing
- Excellent communication and interpersonal skills
- Able to work independently and as part of a team
