Job Summary:
The Case Manager position provides advanced clinical case management.
Job Functions and Duties:
- Assist in the development, implementation and revision of individual treatment plans.
- Continually assesses clients’ goals, strengths and needs in order to meet treatment goals.
- Assure that services provided are specified in the treatment plan and monitor progress toward treatment goals.
- Responsible for administering psycho-education to clients.
- Consult and cooperate with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
- Provide crisis management for clients and make linkages for interventions as appropriate.
- Provide group Activities of Daily Living (ADL) training.
- Maintain client and program records in accordance with applicable standards and regulations.
- Identify skills, benefits and services to support recovery goals.
- Monitor quality of care and progress toward goals.
- Responsible for service coordination or arrangement of services needed.
- Serve as a personal advocate for clients through the review.
- Observe and report any visible changes in clients’ physical, mental, or behavioral status.
- Assists as necessary during client incidents and times of crisis.
- Perform related duties as needed.
Qualifications:
- Master’s or doctorate degree in a human services field and;
- Pending New Jersey state licensure: LSW, LAC, LCADC and;
- A minimum of five years of human service experience and;
