Job Description
As a RN Clinical Liaison, you will be responsible for ensuring the intake and referral processes are performed effectively and timely to meet the needs of patients, families, and referral sources. You will complete admissions processes on referred patients in emergency cases, provide accurate and timely information regarding hospice services, receive calls from referral sources, respond timely, and initiate paperwork to begin Intake/referral process.
Required Skills and Qualifications
- Hospice heart: compassionate, kind, and empathetic to clients, patients, and their families.
- RN license in the state(s) you will be employed.
- Minimum of 2 years of healthcare experience preferred in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
- Known and respected within your community.
