As a member of our team, you will provide personalized care and support to patients in their homes or other settings, developing strong relationships with patients, families, and healthcare teams to ensure seamless care coordination.
Key Responsibilities
- Deliver high-quality patient care and support.
- Develop and implement comprehensive care plans.
- Collaborate with interdisciplinary teams to ensure effective care coordination.
Requirements:
- Certified Nursing Assistant (CNA) license.
- Six months of patient care experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
- Ability to work effectively in a fast-paced environment.
- Excellent communication and interpersonal skills.
