We are seeking a skilled and empathetic Hospice Registered Nurse (RN) Case Manager to join our team at Gentiva Health Services. As a vital member of our interdisciplinary care team, you will be responsible for providing direct patient care services, coordinating care plans, and collaborating with colleagues to ensure seamless transitions and optimal outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a dynamic and fast-paced environment.
**Key Responsibilities**
- Provide direct patient care services, including medication management, wound care, and symptom control
- Develop and implement comprehensive care plans in collaboration with interdisciplinary teams
- Coordinate with physicians, family members, and other stakeholders to ensure effective communication and collaboration
- Participate in ongoing education and training to maintain expertise in end-of-life care and related best practices
- Collaborate with social workers, spiritual care specialists, and other team members to address the holistic needs of patients and their loved ones
Requirements
To be successful in this role, you will possess a current RN license in the state of employment, 1 year of experience in hospice, home health, ICU, geriatric, or oncology, and excellent communication and interpersonal skills. Additional preferred qualifications include experience with electronic medical records, case management software, and palliative care principles.
