Job Summary
We are seeking an experienced and compassionate Registered Nurse to join our team as a Hospice Case Manager RN. In this role, you will be responsible for managing patient care services, collaborating with interdisciplinary teams, and empowering caregivers with education and support.
- Provide direct patient care services to hospice patients and supportive care to patients' families.
- Act as primary liaison between physicians, patients/families, and hospice teams.
- Evaluate patient and family needs and assist in preparing and executing plans of care.
- Collaborate with interdisciplinary teams to provide comfort, peace, and dignity to patients.
Required Skills and Qualifications
To be successful in this role, you will need:
- A current RN license in the state of employment (or eligibility to obtain).
- 1 years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work independently and collaboratively as part of a team.
