Key Responsibilities:
• Provide direct patient care services to hospice patients and supportive care to the patient's family on an on-call basis
• Maintain primary control and professional management of each patient
• Act as the primary liaison between physician, patient/family, and hospice team
• Evaluate patient/family needs and care for each patient
• Complete admissions and pronouncements
• Maintain all assigned patient clinical records
