We're seeking a compassionate and experienced Registered Nurse to join our on-call team as a Weekend On-Call RN - 5pm Friday - 8am Monday Full Time. In this critical role, you will provide direct patient care services to hospice patients and supportive care to the patient's family after normal business hours on an on-call basis. You will maintain primary control and professional management of each patient, acting as the primary liaison between physician, patient/family and hospice team. Your responsibilities will include evaluating patient/family needs, completing admissions and pronouncements, and maintaining all assigned patient clinical records.
**Key Responsibilities:**
• Provide high-quality patient care and support to hospice patients and their families.
• Maintain accurate and comprehensive patient clinical records.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
• Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans.
**Requirements:**
• Current RN license (in the state of requested employment or ability to obtain).
• 1 years of nursing experience in hospice, home health, or oncology preferred.
• Compassionate, kind, and empathetic approach to patient care.
