Job Description
We are seeking a compassionate and experienced Hospice Registered Nurse to join our team as an RN Case Manager. In this role, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities
- Provide direct patient care services, including medication management and wound care
- Assess patient needs and develop individualized plans of care
