The RN Clinical Liaison educates healthcare professionals about hospice services and provides end-of-life solutions committed to clinical excellence, compassion, and dignity when life matters most.
The role involves ensuring effective intake and referral processes, completing admissions processes, providing accurate information about hospice services, and serving as a patient advocate in evaluating and developing plans of care.
Requirements:
Applicants should have a Hospice heart, be compassionate, kind, and empathetic towards clients, patients, and families.
They must hold an active RN license in the state(s) they will be employed in, have at least 2 years of healthcare experience in settings such as hospice, home health, or clinical coordination, and be known and respected within their community.
