About the Role
We are seeking an experienced Registered Nurse (RN) to join our team as a Patient Care Manager RN. As a key member of our interdisciplinary team, you will be responsible for managing and overseeing patient care activities to ensure the delivery of quality patient care.
Your responsibilities will include:
- Maintaining a high-performing hospice program
- Oversight of rescheduled, declined, missed, and reassigned visit requests
- Response to phone referrals/inquiries in a timely and professional manner
- Preparation for, serving on, and facilitating the Interdisciplinary Group (IDG)
Requirements
To be successful in this role, you will need:
- At least three years' experience in direct patient care
- Two years' experience in hospice or home-based care setting
- Graduate of accredited school of nursing; bachelor's degree in nursing preferred
- Current Registered Nurse licensure in state of residence and requested employment
