Responsibilities
As a Hospice RN Case Manager, your primary responsibilities will include:
- Providing direct patient care services to hospice patients and their families
- Collaborating with an interdisciplinary team of caregivers, including LPNs, CNAs, and support staff
- Evaluating patient and family needs and assisting in the preparation and execution of the plan of care
- Maintaining all assigned patient clinical records
