About the Role
We are seeking an experienced RN to serve as a Patient Care Manager. In this role, you will be responsible for overseeing patient care activities, managing staff, and ensuring compliance with regulatory requirements.
Responsibilities:
- Maintain a high-performing hospice program
- Oversee rescheduled, declined, missed, and reassigned visit requests
- Respond to phone referrals and inquiries in a timely and professional manner
- Prepare for, serve on, and facilitate Interdisciplinary Groups (IDGs)
Requirements
To be successful in this role, you will need:
- A minimum of three years' experience in direct patient care
- Two years' experience in hospice or home-based care setting
- A graduate degree in nursing
- Current Registered Nurse licensure
- Demonstrated knowledge of accepted hospice principles and practice
