Position Overview :
We are seeking a Patient Care Supervisor to enhance our quality of care. Reporting directly to the Executive Director or Senior Patient Care Manager, you will play a crucial role in managing and directing patient care activities. Your responsibilities will include teaching and developing associates, ensuring compliance with regulations, and maintaining a high-performing hospice program.
- Oversee rescheduled, declined, missed, and reassigned visit requests, making staffing decisions based on clinical judgment.
- Respond promptly and compassionately to phone referrals / inquiries, triaging medical concerns within your scope of practice.
- Facilitate the Interdisciplinary Group (IDG) by presiding over meetings, reviewing patient cases, and coordinating care documentation.
