Job Summary
We are seeking a highly skilled RN to join our team as a Patient Care Manager. In this role, you will be responsible for maintaining a high-performing hospice program, managing staff, and ensuring compliance with regulatory requirements.
Key Responsibilities:
- Maintain a high-performing hospice program
- Oversee and manage rescheduled, declined, missed, and reassigned visit requests
- Respond to phone referrals and inquiries in a timely and professional manner
- Facilitate Interdisciplinary Groups (IDGs) and coordinate minutes
Requirements and Qualifications
To be successful in this role, you will need:
- A minimum of three years' experience in direct patient care
- Two years' experience in hospice or home-based care setting
- A graduate degree in nursing
- Current Registered Nurse licensure
- Demonstrated knowledge of accepted hospice principles and practice
