Hospice RN Clinical Liaison Responsibilities:
Completes the admissions process on referred patients in emergency cases, as staffing indicates, and for inpatient hospice admissions
Provides accurate and timely information regarding hospice services to anyone inquiring
Serves as patient advocate in evaluating/developing plan of care and obtaining informed consent for hospice
Calling on multiple account types – SNFs, ALFs, Hospitals, and Physicians
Offering in-services and education on the benefits of hospice
Hospice RN Clinical Liaison Requirements:
A Hospice Heart: compassionate, kind, and empathetic to your clients, their patients, and their families
Minimum of 2 years of healthcare experience preferred in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator
Known and respected within your community
