About the Role:
We are looking for a skilled and compassionate Registered Nurse to join our team as a Clinical Liaison. As a key member of our clinical staff, you will play a critical role in educating healthcare professionals about hospice services and providing end-of-life solutions that prioritize clinical excellence, compassion, and dignity.
Responsibilities:
- Educate healthcare professionals about hospice services and provide information regarding hospice care options.
- Complete the admissions process on referred patients in emergency cases and initiate paperwork to begin the intake/referral process.
- Act as a patient advocate in evaluating and developing plans of care, obtaining informed consent for hospice care, and calling on multiple account types - SNFs, ALFs, hospitals, and physicians.
- Offer in-services and education on the benefits of hospice care.
Requirements:
- A RN license in the state(s) where you will be employed.
- Minimum of 2 years of healthcare experience preferred in one of the following settings: hospice, home health, clinical liaison, RN liaison, clinical coordination, discharge planner, hospice case manager, or RN educator.
- Known and respected within your community.
