This role involves ensuring the intake and referral processes are performed effectively and timely, completing admissions, providing information about hospice services, and advocating for patients and their families. As a RN Clinical Liaison, you will be responsible for calling on multiple account types, offering in-services and education on the benefits of hospice, and serving as a patient advocate in evaluating and developing plans of care.
