Key Responsibilities:
- Educate healthcare professionals about Gentiva's comprehensive hospice services
- Develop and implement effective intake and referral processes to meet patient needs
- Collaborate with interdisciplinary teams to provide end-of-life solutions with clinical excellence, compassion, and dignity
- Serve as a patient advocate in evaluating and developing plans of care
Requirements:
- RN license in the state(s) of employment
