We are seeking a Full-Time Licensed Clinical Social Worker to join our interdisciplinary team in Jackson, Ohio. The ideal candidate will have a Master's Degree in Social Work from a CSWE accredited School and licensure as required by the state in which the hospice is located.

- Conduct comprehensive psychosocial assessments to identify needs, goals, interventions, and services
- Develop and implement individualized plans of care based on patient and family needs
