What are the responsibilities and job description for the Hospice NP Clinical Services Provider position at Gentiva?
We're looking for an experienced Nurse Practitioner to join our hospice care team on a per diem basis. As a member of our interdisciplinary group, you'll play a key role in delivering clinical services to our patients and supporting the diagnosis and prognosis process.
About the Role
In this role, you'll conduct face-to-face encounters with patients to gather clinical information, communicate patient findings to the physician-led team, and make recommendations based on your assessment. You'll also ensure regulatory compliance in F2F visit completion, working closely with our physician-led team to deliver high-quality care to our patients.

Requirements
To be successful in this role, you'll need:
- A Master's or higher degree in Nursing
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body
- A current state license as a registered nurse
- A current state license or certificate as an advanced practice nurse
- A current national certification as a Nurse Practitioner (ANCC or AANP)
- A collaborative agreement in place
