Responsibilities
As a Nurse Practitioner within our Hospice team, you will be responsible for the following tasks:
- Conducting medical assessments and developing care plans
- Gathering clinical information to support the hospice diagnosis and prognosis
- Communicating findings to the hospice physician and making recommendations to the IDG based on your assessment
- Collaborating with other healthcare professionals to ensure seamless care delivery
- Managing patient symptoms and improving quality of life through evidence-based practices
Requirements
To be successful in this role, you will need to possess the following qualifications and attributes:
- Master's degree in Nursing from an accredited institution
- Completion of a graduate-level nurse practitioner education program accredited by a national accrediting body
- Current state licensure as a registered nurse
- Current national certification as a Nurse Practitioner (ANCC or AANP)
- Collaborative agreement in place
