Nurse Practitioner Hospice
We are seeking a skilled Nurse Practitioner to join our team in providing high-quality hospice care. As a vital member of our interdisciplinary group, you will be responsible for conducting Face-to-Face (F2F) encounters and gathering clinical information to support patient eligibility.
Responsibilities:
- Perform physical assessments for F2F encounter visits within regulatory time frames established by CMS.
- Provide clinical findings, including review of medical treatment plans, diagnostic tests, lab results, and medication lists to the certifying physician.
- Makes recommendations to the IDG based on F2F assessment findings.
Requirements:
- Master's or higher degree in Nursing.
- Graduate-level nurse practitioner education program accreditation by a national accrediting body.
- Current state license as a registered nurse.
- Current state license or certificate as an advanced practice nurse.
- Current national certification as a Nurse Practitioner (ANCC or AANP).
